A New Standard of Care: The Integration of Behavioral Health and Medicine

People living with serious mental illnesses are dying 25 years earlier than the rest of the population, in large part due to unmanaged physical health conditions. There is a need to align integration of primary health care and mental health care with developments around the medical home.  Integrated Care is on the rise around the county and is an effective way of delivering care in the context of chronic disease.  Organized to address the implementation of Senate Bill 2829 in Mississippi, this session will address Mississippi Health Home Integration and what is happening around the county with Integrated Care.  Core elements of the SB 2829 will be reviewed, and evidenced implementation approaches will be discussed. Other key elements of the Integrated Care service concepts will be discussed.
